Getting disconnected constantly and No calling is being made.

I am a very new user to goautodial. I have setted up everything.

Server, voip, campaign etc. Now when a agent logs in the softphone eyebeam gets a call but once call is picked up it gets disconnected showing(hang up).
Please guys assist me with this.

can you post the asterisk cli log, while gettign this issue.
command to get in to asterisk cli
asterisk -vvvvvr

also run the below command and post back the result

screen -list
