Custom Fields - Radio or Select Dialog

Description
The custom fields now have essential functionality after downloading and the latest ce3, but I am unable to create "radio" or "select" fields.
Text fields are working, but at times when you try to relocate them it will not save. Delete and recreate seems to solve this part for now.

Additionally in 32 or 64 bit (i have tested both), not sure if you want me to open a separate issue, but if you create extra fields and try to import you always get errors such as:
Error Number: 1054
Unknown column 'title' in 'field list'
INSERT INTO custom_1000 SET lead_id='4',company='T A S INC',title='T A S INC',number_emps='11',curr_pr='ADP';

Issue occurs when attempting to import leads to new list with custom fields. The OEM "load me some leads" list works correctly.

Fix is available via update. Run "yum update" to download and install the fix.
