adding leads

I hope when i send this mail... any1 doesn't ask my conf as it is of no use here.

While i m uploading the leads its showing ERROR: The file does not have the required number of fields to process it.
FYI: i have read the staring guide and copy pasted the way to do it from there

The fields were in capital first, it showed the error message therefore changed it to small.
