Please help, unable to get any dialing out

I've been banging my head on this for 2 days and can't figure it out. Unable to get ANY dialing out. Fresh 3.0 install. Default campaign as per starting guide. For carrier selection it's blank dial code. Default justgovoip.com config:

My version number is 3.0 and under Account Entry I added fromuser=USERIDFROMCARRIER and it started working. However, I can not get caller ID to broadcast properly. It is set in campaign, but outgoing calls show default carrier CID.
