Problem in agent logging Please help

I installed the new version 3.3 goautodial
and whenever I have a preblem with an agent , the campaign is off and it gives the identifier of the agent.
You'll find an image that decribe my problem.
Please help me and even I delete the same user and the phone and I said the problem remains as it is.

I have exactly the same issue:
Clean install, did GIT update. (followed all instructions, permissions update etc)
I ran; update_server_ip
Removed all users and re-created all users and extensions same issue.
Created new Campaign; same issue.
Re-created all user/extensions and new campaign; same issue
