Campaigns settings dalprefix/carrier entry

Description
Chris,
We need to replace the dial prefix entry in the campaigns settings to a drop down menu for the available active carriers. The last entry should be "manual" so the user can override or choose their own dial prefix.

Please note that only active carriers should appear on the Carrier to use for this Campaign.

Thanks for the update. It Works perfectly. Only the active carriers shows on the dropdown menu of "Carrier to use for this Campaign."
