Campaigns settings dalprefix/carrier entry

Description
Chris,
We need to replace the dial prefix entry in the campaigns settings to a drop down menu for the available active carriers. The last entry should be "manual" so the user can override or choose their own dial prefix.
This includes the campaigns creation wizard (step 3).

Done with this feature... Kindly test it on our .125 or PNAP test servers...

I made some test and my active Carriers are not showing on "Carrier to use for this Campaign". Please see the screenshot below. Thanks!

Please note that only active carriers should appear on the Carrier to use for this Campaign.
