Can't Login, Kamailio doesn't start, Asterisk start manually after reboot

Hello,
I have got 9 screens running with GoAutoDial v4. But I am facing all sorts of problems. When I open the login.php I see in the file below
tail -f /var/log/php-fpm/www-error.log

When I try to login apart from the fact that the password not working I see the lines below right after the above logs
